REQUEST FOR PROPOSALS (RFP) FOR DESIGN BUILD SERVICES
MENTAL HEALTH KOKUA – KAUAI RESTORATION PROJECT

Mental Health Kokua (Owner), a non-profit organization, is soliciting sealed proposals from qualified Hawaii-licensed contractors to furnish all labor and materials to connect Kauai rehabilitation of 6330 Koukalaka Place, Kapaa. Scope of work to include: replacing flooring, updating kitchen, refurbishing ADA bathrooms, replacing jealousy windows with vinyl sliders, check/increase electrical box capacity and replace light fixtures, replace windowsills, replacing 9 interior closet’s sliding doors, 2 exterior doors, and interior/exterior prep/painting.

Proposals must be received by 4:00 p.m on November 18, 2025.

A site visit and inspection of the Project will be held on October 29, 2025 at 10:00 am. The site address is 6330 Koukalaka Place, Kapaa, HI 96746. All prospective Bidders for the Project are strongly advised to attend the site visit and inspection which will address any questions regarding the scope of work for the Project.

The Bid Specifications Form and all requested information and documentation must be submitted via email to Cordula Diaz, cdiaz@mhkhawaii.org.

Bids submitted by interested parties for the Project and received by the deadline will be considered and opened at 680 Iwilei Road Suite 600, Honolulu HI
96817-5393 on,
November 18, 2025 (which is subject to change).

This project is being funded in whole or in part with federal funds from the Department of Housing and Urban Development (HUD) Community Development Block Grant (CDBG) Program, as administered by the County of Hawaii. The successful awardee shall be responsible for compliance with all current County, State, and Federal laws, rules and regulations, including without limitation, those mandated by HUD regarding the CDBG Program.
