VARIANCE APPLICATION NO. WW 923

Pursuant to Hawaii Revised Statutes Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested person(s) regarding the following. Ms. Colleen McCarthy has applied for a variance for the maximum of five (5) years from Hawaii Administrative Rules, Chapter 11-62, “Wastewater Systems.”
Mr. Matt Volk, Volk LLC, is the authorized agent to act for the applicant. The variance request is for the construction of a new individual wastewater system to be installed at 3625 B Lawaiuka Road, Lawai, Hawaii 96765 and TMK: (4) 2-5-006: 032.
