Pursuant to Hawaii Revised Statutes Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested person(s) regarding the following. Ms. Crystal Laffey has applied for a variance for the maximum of five (5) years from Hawaii Administrative Rules, Chapter 11-62, “Wastewater Systems.”
Mr. Matt Volk of Volk LLC is the authorized agent to act for the applicant. The variance request is for the construction of a new individual wastewater system to be installed at 4565 Mamane Street, Kapaa, Hawaii 96746 and TMK: (4) 4-5-009: 039.
