VARIANCE APPLICATION NO. WW 886

Pursuant to Hawaii Revised Statutes Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested person(s) regarding the following. Mr. Ferdinand Pabustan has applied for a variance for the maximum of five (5) years from Hawaii Administrative Rules, Chapter 11-62, “Wastewater Systems.”
Ms. Sarah Broward, Lox Hawaii, LLC, is the authorized agent to act for the applicant. The variance request is for the construction of a new individual wastewater system to be installed at 5137 Lokene Street, Kapaa, Hawaii 96746 and TMK: (4) 4-6-036: 135.
