VARIANCE APPLICATION NO. WW 603

Pursuant to Hawaii Revised Statutes (HRS) Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested persons regarding the following. Mr. Mark Murphy has applied for a variance for the maximum of five (5) years from sections 11-62-32 and 11-62-34(c)(1)(C) of Hawaii Administrative Rules, “Wastewater Systems.”
Ms. Sarah Broward, Designer, Lox Hawaii, LLC, is the authorized agent to act for the applicant. The variance request is for the construction of a new individual wastewater system to be installed at 4287 Anini Road, Kilauea, Hawaii 96754 and TMK (4) 5-3-007:011.
