VARIANCE APPLICATION NO. WW 594

DOCKET NO. 20 – VWW – 30

Pursuant to Hawaii Revised Statutes (HRS) Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested persons regarding the following. Mr. Kenneth Terheggen has applied for a variance for the maximum of five (5) years from sections 11-62-32 and 11-62-34(c)(1)(C) of Hawaii Administrative Rules, “Wastewater Systems.”
Ms. Sarah Broward of Lox Hawaii LLC is the authorized agent to act for the applicant. The variance request is for the installation of a new individual wastewater system to be installed at 174 Ohana Street, Kapaa, Hawaii 96746 and TMK (4) 4-1-004: 028.
