DESCRIPTION OF SERVICES
Provide services for a care navigation program aimed at improving the continuum of care for kupuna (elderly) and their caregivers on Kauai. The goal of this program is to improve health outcomes for kupuna, recruit and retain healthcare jobs, and create overall cost savings for high-risk patients in the community.

PROGRAM FUNDING:
Funded by a State of Hawaii operating grant-in-aid. Successful bidder shall be responsible for compliance with all applicable County, State and Federal laws, rules and regulations. Program duration is one year.
