VARIANCE APPLICATION NO. WW 678

Pursuant to Hawaii Revised Statutes (HRS) Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested persons regarding the following. Ms. Jean Nadatani has applied for a variance for the maximum of five (5) years from section 11-62-32 of Hawaii Administrative Rules, “Wastewater Systems.”
Ms. Sarah Broward, Designer, Lox Hawaii LLC, is the authorized agent to act for the applicant. The variance request is for the construction of a new individual wastewater system to be installed at 4539A Akemama Road, Lawai, Hawaii at TMK (4) 2-5-003: 006.
