VARIANCE APPLICATION NO. WW 950

Pursuant to Hawaii Revised Statutes Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested person(s) regarding the following. Mr. Frederick Tjarks has applied for a variance for a maximum of five (5) years from Hawaii Administrative Rules, Chapter 11-62, “Wastewater Systems.”
Ms. Sarah Broward of Lox Hawaii LLC is the authorized agent to act for the applicant. This variance request is for the construction of a new individual wastewater system to be installed at 5309 Kihei Road, Kapaa, Hawaii 96746 and TMK: (4) 4-1-010: 013.
