You are hereby notified that a Petition for Adoption has been filed in the Family Court, Fifth Circuit, State of Hawaii in FC-A No. 21-1-0022. Information regarding the children to be adopted is as follows:

The Petition alleges that your consent to the adoption of the above-named children by the named Petitioner is not required and may be dispensed pursuant to section 578-2(c) of the Hawaii Revised Statutes, as amended.
YOU ARE HEREBY SUMMONED to appear at 8:00am on May 17, 2022 at the Family Court of the Fifth Circuit (Courtroom 1) located at 3970 Kaana Street, Lihue, Kauai, Hawaii 96766 to answer to the Petition for Adoption.
If you fail to appear at the hearing on that date and time and at the place noted above or if you fail to file a written response to the allegations reflected in the Petition for Adoption, further action may be taken including the granting of the adoption without further notice to you.
You are further notified that the child, the adoptive parent, and the natural parent have rights under HRS Section 578-15 regarding the confidentiality of adoption records after the children reach age 18.
