DE COSTA, filed a Complaint against you in Civil No. RC-18-1-0516 in the District Court of the Fifth Circuit, State of Hawaii, praying for judgment for sums due and owing for nonpayment of $5,322.30, interest, costs, attorney’s fees and for other proper relief.

YOU ARE HEREBY SUMMONED to appear in the Courtroom of the Honorable Presiding, Judge of the above-entitled Court, Kauai Hawaii, on Monday, the 14th day of October, 2019, at 8:00 a.m. of said day or to file an answer or other pleading and service it before said day upon CRAIG A.
