VARIANCE APPLICATION NO. WW 640

Pursuant to Hawaii Revised Statutes (HRS) Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested persons regarding the following. Mr. Lawrence Billimek has applied for a variance for the maximum of five (5) years from section 11-62-34(c)(1)(C) of Hawaii Administrative Rules, “Wastewater Systems.”
Ms. Sarah Broward, Designer, Lox Hawaii LLC, is the authorized agent to act for the applicant. The variance request is for the construction of a new individual wastewater system to be installed at 4405 Aku Road, Hanalei, Hawaii 96714 and TMK (4) 5-5-010: 026.
