VARIANCE APPLICATION NO. WW 546

Pursuant to Hawaii Revised Statutes (HRS) Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested persons regarding the following.Mr. Derek Hagglof has applied for a variance for the maximum of five (5) years from section 11-62-32, Hawaii Administrative Rules (HAR) “Wastewater Systems.” Mr. Kevin Fujimoto, Fujimoto Engineering LLC, Lihue, Kauai is the individual authorized to act for the applicant. This variance request is for the installation of an individual wastewater system to be installed within 1,000 feet from a public drinking water well for the property located at 3657 Ilima Place, Kalaheo, Hawaii 96741 and TMK (4) 2-5-011: 033.
