VARIANCE APPLICATION NO. WW 534

Pursuant to Hawaii Revised Statutes (HRS) Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested persons regarding the following. Mr. Christopher Jordan, Koloa, Kauai has applied for a variance for the maximum of five (5) years as stated in the variance application from Hawaii Administrative Rules (HAR) sections 11-62-32.
Mr. Wayne Wada, P.E., Esaki Surveying & Mapping, Inc., Lihue, Kauai, is the individual authorized to act for the applicant. The variance request is for a new individual wastewater system to be installed at 4516 Lawai Road, Koloa, Kauai, Hawaii 96756 and TMK (4) 2-6-003: 007.
